Anyone can book home visit for new Patient and deliver the form via any delivery means listed below.
Anyone can book a Home collection for existing patients, and forms can be provide via any means listed below. Examples who who books visits:
QML Prefer to physical form from patients on day of collection. Forms can be received via any means for new or existing patients though. Email, fax, provided on collection by patient, or staff.
Booked 7am to 10am, Patients in Any location can have fasting tests booked. Limit on how many patients can be booked per day.
Booked between 7am to 10am, Patients in Any location can have withholding tests. Limit on how many patients can be booked per day.
Booked for 8am ONLY. Bookings marked as URGENT. Any Patient location can Limit on how many per day.
Booked between 7am and 3pm. No timeslots given.
Patients are flagged in system for missed tests. If a patient misses three tests they are removed from Home Visits collections. Collection Co-ordinators and Doctors are advised.
INR Home Visits bookings handled by the Warfarin Paperwork team.
Some Tests not eligible to be collected at home. This will be advised at time of booking request via phone or email. Team references the Collection Manual for home collection eligibility
Home Visits collections with Out of Pocket costs are collected and then invoiced to patients for Payment. Test costs needed to be linked to form from the Collection Manual.

Not able to be booked via webform - as these need to be booked immediately for the first available appointment the following day. Direct to call centre.
Needs Collection Lead approval to be booked - direct to contact centre.
Rule 3 patients with scheduled tests are booked in advance by Home Visits Staff. Actioned monthly. Patients need to provide new Rule 3 Card every 6 bookings. Maximum of 6 bookings Per Rule 3 form per 6 month period.
